Blood Sugar, Diabetes and Memory: What the Evidence Shows

Type 2 diabetes is associated with higher rates of cognitive decline. The relationship is well documented, the mechanisms are partly understood, and the implications are practical.

Glucose regulation has a substantial literature in relation to cognition, and it is one of the areas where the association is consistent enough to take seriously.

What the research indicates

Type 2 diabetes has been repeatedly associated with higher rates of cognitive decline and dementia diagnosis compared with people without diabetes. Reviews of modifiable dementia risk factors commonly include diabetes among the factors identified.

Some studies have also reported associations between elevated blood glucose below the diabetic threshold and cognitive outcomes, suggesting the relationship may not begin sharply at a diagnostic cut-off. This work is less settled.

Proposed mechanisms

  • Vascular damage. Diabetes damages both large and small blood vessels, including those supplying the brain. Much of the cognitive association may run through this route, which overlaps heavily with the blood pressure story.
  • Insulin signalling in the brain. Insulin receptors are present in brain regions including the hippocampus, and insulin resistance has been proposed to affect neuronal function. This is an active research area rather than settled science.
  • Inflammation and advanced glycation end-products, both elevated in poorly controlled diabetes.
  • Hypoglycaemia. Severe low blood sugar episodes, which can occur as a consequence of treatment, have themselves been associated with cognitive risk. This is one reason glycaemic targets in older adults are often set less aggressively.
An important nuance

Tighter control is not straightforwardly better for the brain. The relationship appears more complex, with both sustained hyperglycaemia and severe hypoglycaemic episodes carrying risk. Targets are individualised, and that individualisation is a clinical decision.

Short-term effects

Separately from long-term risk, acute glucose fluctuations affect cognition in the moment. Hypoglycaemia impairs attention, working memory and judgement, and severe episodes can cause confusion. Marked hyperglycaemia is also associated with reduced attention and processing speed in some studies.

For someone with diabetes noticing cognitive fluctuation through the day, glucose variability is worth discussing with their care team as a possible contributor.

What is worth doing

For people without diabetes, the measures associated with reduced risk are the familiar ones — they are the same measures that appear throughout this site because the underlying physiology overlaps:

  • Regular physical activity, which improves insulin sensitivity independently of weight change
  • Dietary patterns emphasising whole foods, fibre, legumes and whole grains over refined carbohydrate and added sugar
  • Weight management where clinically relevant
  • Adequate sleep — sleep restriction has measurable effects on glucose regulation
  • Screening, since type 2 diabetes and prediabetes are frequently asymptomatic for years
